How protected health information may be used, shared, and accessed

This notice describes how protected health information may be used and disclosed by Jennifer Wisser-Stokes Counseling LLC and how you can get access to that information. Please read it carefully.
This notice explains
This notice applies to records and information created or maintained by Jennifer Wisser-Stokes Counseling LLC in connection with services. It explains general privacy practices, client rights, and the practice's responsibilities under applicable privacy rules.
How protected health information may be used and disclosed
For treatment
The practice may use or share protected health information as needed to provide, manage, or coordinate care. This can include reviewing records, communicating within the practice about care, and coordinating with other providers when appropriate authorization or legal permission exists.
For payment
The practice may use or disclose information as needed for billing, payment collection, account management, superbills, and related financial activity.

For administrative and support functions
Practice staff and service providers who support scheduling, billing, records, and routine practice operations may have access to information as needed to do their work, subject to privacy protections and legal requirements.
Uses and disclosures that usually require written permission
Uses or disclosures not described in this notice will be made only with written authorization when authorization is required by law. Clients may revoke a written authorization in writing, except to the extent the practice has already acted on it.
Your rights
Right to access records
You may request access to records or a summary of records, including an electronic copy when available, subject to limited exceptions allowed by law.
Right to request limits
You may request limits on certain uses or disclosures. The practice will consider such requests but is not always required to agree.
Right to request confidential communication
You may request that the practice communicate with you in a particular way or at a particular location when reasonable.
Right to request amendment
If you believe information in your record is incomplete or incorrect, you may request an amendment.
Right to an accounting of certain disclosures
You may request an accounting of certain disclosures made outside the most routine permitted categories.
Right to a copy of this notice
You may request a paper or electronic copy of this notice at any time.
Right to have someone act for you
If someone has legal authority to act on your behalf, such as a parent, legal guardian, or health care representative, the practice may work with that person as allowed or required by law.
Right to file a complaint
You may file a complaint with the practice or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated.
Special notes for parents and minor clients
When the client is a minor, privacy rights may involve both the child and the parent or legal guardian. Jennifer Wisser-Stokes Counseling LLC works carefully to balance the child or teen's therapeutic privacy with the legal rights and responsibilities of parents or guardians.

In some situations, a parent or guardian may have rights related to records and treatment decisions. In other situations, access or disclosure may be limited by law, court order, safety concerns, or clinical judgment allowed under applicable rules.
The practice's duties
Jennifer Wisser-Stokes Counseling LLC is required to maintain the privacy of protected health information, provide this notice, follow the terms of the notice currently in effect, and notify affected individuals if a reportable breach of unsecured protected health information occurs.
